Birth
The birth declaration must be made to the Civil Registry Office of the municipality where the mother is resident, or where the birth took place, within 10 days of the birth. Alternatively, it may be made within 3 days of the birth at the health unit of the hospital or nursing home where the birth took place.
When declaring the birth, in the case of both a child born in wedlock and a child born out of wedlock, the child's surname must be composed with the parents' surnames in the order chosen by the parents by mutual agreement. If there is no agreement, the parents will have to apply to the Ordinary Tribunal for a court order.
Only in the event of agreement between the parents themselves may only one surname be assigned.

the original birth certificate issued by the hospital;

a valid identity document. For foreign nationals a passport must be submitted. Foreign citizens who do not speak Italian must be accompanied by a person who can act as their interpreter.
